Yes, you must provide your social security card and government issued photo id, such as a driver’s license or a passport. In addition, the first and last name on the photo id and social security card must match exactly. Students are encouraged to apply for replacement or corrected social security card and or driver’s license as soon as possible to avoid a possible delay in taking the State Certification exam.
